INVITATION TO TENDER/EXPRESSION OF INTEREST FOR 2018 CAPITAL PROJECTS
The Federal Teaching Hospital Ido-Ekiti, a tertiary health of her capital projects in the FY2018 and in compliance with the Public Procurement Act, 2007 hereby invites all interested, qualified and reputable contractors with relevant experience and track record to tender for the following projects under 2018 Capital Projects.

CATEGORY A: WORKS
LOT 1: Construction of Modern Adult Accidents and Emergency Building
LOT 2: Renovation of Medical Rehabilitation Department Building
LOT 3: Renovation and Upgrading of Operating Theatre Building
CATEGORY B: GOODS
LOT 4: Procurement of Furniture and Beddings for new 4-Suite Modular Theatre & 4 Sub-Specialty Wards Building
LOT 5: Procurement of Air Conditioners, Computers etc. for New 4-Suite Modular Theatre & 4 Sub-Specialty Wards Building
LOT 6: Procurement and Installation of 6 Set Dialysis Equipment
LOT 7: Procurement of Menstrual Cups for Women
LOT 8: Procurement and Installation of 250KVA Perkins Diesel Generators
LOT 9: Procurement and Installation of Theatre and Laboratory Equipment
CATEGORY C: SERVICES
LOT 10: Consultancy Service for Training of Theatre and Laboratory Staff
LOT 11: Non-Consultancy Services for Medical Outreach in Federal Teaching Hospital Ido-Ekiti, Ekiti State.
B. ELIGIBILITY REQUIREMENT
a. Evidence of up-to-date company registration with Corporate Affairs Commission, by inclusion of Certificate of Incorporation, Form CAC2 and CAC7.
b. Tax clearance certificate for the last three (3) years (i.e. 2015, 2016 and 2017) valid till 31st December 2018, with minimum average annual Turnover of 20million.
c. Certificate of National Pension Commission (PENCOM) valid till 31st December, 2018.
d. Certificate of Compliance issued by the Industrial Training Fund (ITF) valid till 31st December, 2018.
e. Evidence of Registration on National Database of Contractors, Consultants and Service Providers by inclusion of Interim Registration Report (IRR) from BPP expiring on 1/1/2019 or Valid Certificate issued by BPP.
f. Evidence of current Nigerian Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2018.
g. Company Audited Accounts for the last three (3) years (i.e. 2005, 2016 and 2017).
h. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of award, Valuation Certificates, Completion Certificates and photographs of the projects.
i. Evidence of financial capability of the company with guarantee/reference letter from reputable Bank.
j. Detailed Company profile to include the names, phone numbers, curriculum vitae and credentials and Technical and Professional Staff.
k. Evidence of qualification of at least three (3) key technical personnel with relevant professional bodies such as COREN, ARCON, QSRBON, CORBON, etc. (For Category A)
l. Sworn affidavit certifying that:
i) All the required documents submitted are both GENUINE and UP-TO-DATE
ii) None of the Directors of the company has never been convicted by court of law.
iii) Evidence of Sworn Affidavit disclosing whether or not any officer of the relevant committees of the Federal Teaching Hospital, Ido-Ekiti, or Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars.
iv) The company is not bankruptcy or a party to liquidation proceedings in any court of law.
m. List of Plants/Equipment with proof of Ownership/Lease (where applicable).
n. All documents for submission must be transmitted with a covering/forwarding letter under the Company/Firm’s letter head paper bearing among others, the Registration Number (RC), Contact Address, Telephone Number (Preferably GSM) and E-mail Address. The letter head paper must bear the names and nationalities of the Directors of the company at the bottom of the page duly signed by authorized officer of the firm.
The valuation of the received documents shall be strictly based on the requirement specified above.
C. COLLECTION OF TENDER DOCUMENTS
The tender documents are to be collected from Procurement Office between the working hours of 08.00am and 04.00pm daily from the date of this publication upon the payment of non-refundable processing Tender Fee of N10,000.00 into the Federal Teaching Hospital, Ido-Ekiti’s REMITA’s Account in any Commercial Bank and presentation of the copy of Hospital’s receipt. Please, note that bidders for consultancy services will not collect tender documents at this stage until after pre-qualification.
D. SUBMISSION OF DOCUMENTS
Completed tender documents should be two hard copies (Marked “Original” & “Copy”) each for both Technical & Financial Bids with Soft copy of Financial bid only, packaged in two separate wax sealed envelopes clearly marked at the top right corner of each envelope stating the title of the project i.e. “TECHNICAL BIDS FOR LOT 1:” & “FINANCIAL BIDS FOR LOT 1:” and at the reverse side the name of the company should boldly be written. Both envelopes shall then be put in a large envelope clearly marked at the top right corner stating the title of the project and be left in the appropriate Tender Box at Procurement Office, to be addressed to:

The submission should be made not later than 12:00 noon on Monday 6th August, 2018. Late application will be returned unopened. Please make sure you signed Bid Submission Register at Procurement Office Ground floor Administrative Building FTH, Ido-Ekiti, as Federal Teaching Hospital, Ido-Ekiti will not be held responsible for misplaced or wrongly submitted Bids.
E. CLOSING DATE AND OPENING
The closing date for submission of Tender Documents is on/before 12:00noon on Monday 6th August, 2018.
Only technical bids and Expression of Interest (EOI) will be opened the same date at 12.00noon on Monday 6th August, 2018 at the Hospital’s MINI CONFERENCE HALL (Old School of Nursing Hall), in the presence of bidders in attendance, interested general public, and Non-governmental Organizations (NGOs). Only those whose technical Bids are successful will be invited at a later date for opening of the financial bid. Also, only pre-qualified consultants at the EOI stage would be invited for collection of Request for Proposals.
• Bids/EOI must be in English Language and signed by officials authorized by the bidders
• Bidders should not bid for more than two (2) lots.
• This publication is for information purpose only and does not constitute an offer by the Hospital to transact with any party for the project, nor does it constitute a commitment or an obligation on the part of the Hospital to award the contract to any contractor or agent.
• The Hospital will not be responsible for any cost or expenses incurred by any interested company(ies) in connection with any response to this invitation and/or preparation or submission of the documents.
• FTH, Ido-Ekiti also reserves the right to invoke the provision of section 28 of Public Procurement Act (PPA) 207 without incurring liabilities to the bidder.
• The Hospital’s Tender Board is not duty bound to short-list any Contractor or award the contract to any Contractor or Agent.
• All bidders, relevant Civil Society Organization and general public are invited to attend the exercise.
