INVITATION TO TECHNICAL AND FINANCIAL BIDS
1.0 INTRODUCTION
The Federal Medical Centre, Nguru is desirous of undertaking the Supplies of Drugs, Consumables and other items in view of the above, the Centre is inviting interested Companies/Firms to tender.
2.0 PROJECT DESCRIPTION
LOT 1: Supply of Drugs

LOT 2: Supply of Surgical and Anesthetics consumables
LOT 3: Supply of Physiotherapy consumables
LOT 4: Supply of Radiology consumables
LOT 5: Supply of Information Management System consumables
LOT 6: Supply of Dental consumables
LOT 7: Supply of Ophthalmology consumables
LOT 8: Supply of Laboratory consumables
3.0 SCOPE OF PROJECT
The scope of projects are detailed in the bidding documents which shall be issued to prospective bidders.
4.0 ELIGIBILITY REQUIREMENTS
Prospective bidders must possess the under-listed requirements to be eligible to participate in the procurement:
i. A certified true copy of Certificate of Incorporation with the Corporate Affairs Commission (CAC) and Article of Association.
ii. Detailed Company Profile.
iii. Evidence of registration on National Database of Contractors, Consultants and Service Providers by inclusion of Interim Registration Report (IRR) issued by the Bureau of Public Procurement (BPP).
iv. A copy of the Company’s Audited Accounts of the preceding three years 2015, 2016 & 2017.
v. Evidence of Tax Clearance Certificate for 2015, 2016 & 2017 corresponding with cleared annual turnover by inclusion of photocopy.
vi. Evidence of Compliance with the Pensions Reform Act, 2004 by inclusion of Compliance Certificate from the National Pension Commission (PENCOM).
vii. Verifiable evidence of financial capability/banking support to handle the project.
viii. Evidence of experience in at least five (5) jobs of similar nature and complexity executed within the last five years with verifiable letters of contract awards and certificates of job completion.
ix. Verifiable evidence of experience and relevant qualifications of key personnel.
x. Evidence of compliance with the Industrial Training Fund Amendment Act 2011 by inclusion of Compliance Certificate from the Industrial Training Fund (ITF).
xi. Evidence of compliance with National Social Insurance Trust Fund by inclusion of Compliance Certificate from the National Social Insurance Trust Fund (NSITF).
xii. Evidence of registration of Pharmaceutical premises with Pharmacist Council of Nigeria.
xiii. Current sworn affidavit disclosing:
a. That the company is not in receivership, insolvent or bankrupt.
b. That the company does not have any Director who has ever been convicted in any court in Nigeria or any other country for criminal offence in relation to fraud or financial impropriety.
c. Whether or not any officer of the Federal Medical Centre, Nguru is a former or present Director, shareholder or has any pecuniary interest in the bidder.
d. That all documents submitted for the bidding are true and correct in all particulars.
5.0 COLLECTION AND SUBMISSION OF BID DOCUMENTS
Interested and competent bidders are requested to collect bid documents from the office of Head of Procurement, Dr. Bello Kawuwa Administrative Block, Federal Medical Centre, Nguru on payment of non-refundable fee of N10,000.00 (Ten Thousand Naira) only for each Lot.
Prospective bidders shall enclosed their Technical document in separate envelope (marked Technical) and the Financial Bid in another envelope (marked Financial) and the two separate envelopes shall be enclosed in one sealed outer envelope clearly marked “INVITATION TO TENDER – Lot No. Submissions should be addressed to:

The completed and sealed bid documents should contain the bidder’s name on the reverse side of the envelope and be deposited in Tender Box in the Office of Head of Procurement, Dr. Bello Kawuwa Administrative Block, Federal Medical Centre, Nguru between the hours of 8:00am to 4:00pm Monday to Friday.
6.0 CLOSING DATE AND OPENING
Submission of bid documents closes on Monday 16th April, 2018 at 12 noon. Any bid received after the stated date and time shall not be considered. The submitted Technical bids will be opened on the same day, at 12 noon at the Conference Room, Dr. Bello Kawuwa Administrative Block, Federal Medical Centre, Nguru. Bidders/Company representatives and interested members of the public including relevant professional bodies, Civil Society and Non-Governmental Organizations are hereby invited to witness the public opening of the Technical Bids. Successful bidder(s) would be invited for the opening of their Financial Bids at a later date after the Technical Evaluation. The Financial bids of unsuccessful bidders would be returned to them un-opened.
7.0 PLEASE NOTE THAT:
i. Certificates may be referred to the respective issuing Authority(ies) for verification and authentication.
ii. Technical and Financial bid enclose in one and same single envelope that will expose the Technical and Financial bids at the same time of opening shall be disqualified.
iii. Submissions that do not meet up with the requirements specified above shall be rejected.
iv. The Evaluation of bids shall be strictly based on the requirements specified above.
v. This advertisement is published for information purpose, and should not be construed as a commitment or obligation on the part of the Federal Medical Centre, Nguru to award these contracts.
vi. The Federal Medical Centre, Nguru is not bound to consider any bid and reserves the right to reject all bids at any time prior to the acceptance of a bid or cancel the procurement proceeding in the public interest.
